CREDITOR'S NOTICE TO BORROWER: The Contractor will present this Certificate to you for signature AFTER the work and/or the goods have been satisfactorily completed and delivered. DO NOT SIGN this Certificate until you are completely satisfied that the Contractor has carried out all of his/her obligations to you. The selection of a Contractor, the acceptance of materials used and work performed is YOUR responsibility. The Creditor does not guarantee the material or workmanship, or inspect the work performed.

NOTICE TO OWNER

Do not sign this completion certificate or any agreement stating that you are satisfied with the entire project before this project is complete. Home repair contractors are prohibited by law from requesting or accepting a certificate of completion signed by the owner prior to the actual completion of the work to be performed under the home repair contract.

The undersigned Contractor certifies that:

The goods and services provided are in accordance with the Contract.

The project has been completed in accordance with the Contract and to the satisfaction of the buyers. The buyers have not been given or promised any payment, rebate, or anything of value as an inducement to enter into this transaction.

The buyers signed this certificate after completion of the improvement, and all signatures on this certificate are genuine.
